Complete this form within 30 minutes of a customer reporting fraud on their deposit account

a. Add a Lockout Flag with the Special Handling description (remove release date)
i. Choose "No Withdrawals" and the Special Handling description should say:
"No withdrawals lockout – do not remove this lockout, contact Washington Treasury" (or applicable treasury coordinator team)
b. Add a Warning Flag with the See Account Note description (remove release date)
i. Include the note "Potential Fraud: Contact [email protected]"
1. Insert the treasury coordinator team email handling the instance.
A lockout flag blocks all outgoing funds unless the client explicitly approves the transaction
a. To lockout Prime/Express, create a JIRA ticket and call Treasury Servicing (877-423-9742).
A lockout stops all users from accessing their accounts or any online information
a. Digitally save/retain copies of any fraudulent items in a shared folder
